Insurance Information

As a courtesy we will contact your insurance company to obtain your benefit details prior to your first appointment. Most insurance companies offer in and out of network benefits. In network benefits are provided by a Physician or Provider who is contracted with the health insurance company to deliver services to plan members for specified pre-negotiated rates. Out of Network benefits are provided when a Physician or Provider is not contracted with the health insurance plan. In the event that we are out of network with your insurance provider, we offer payment options to help you get the services you need.

Billing

Patients can expect to receive an Explanation of Benefits (EOB) from their insurance company within 3-4 weeks after services have started.

Should you have any questions regarding your bill please call, (877) 856-2279. Our billing department will be happy to assist you.

If you need physical or occupational therapy after an automobile accident or a work related injury, you will need to provide the name, phone number and address of the insurance carrier, as well as the claim number, and date of injury prior to your first visit. We also require your own personal health insurance information in the event your claim benefits do not cover your care.
